What is an Anal Fissure?
An anal fissure is a small tear in the thin, moist tissue (mucosa) that lines the anus. While they are incredibly common, they can cause significant pain and bleeding during bowel movements feel like “passing shards of glass.” Fissures are very common and are often confused with hemorrhoids. They can occur in both men and women equally, most often in the back midline (85–90%), less commonly in the front midline (10–15%). Rare fissures on the sides may indicate underlying conditions like Crohn’s disease, ulcerative colitis, or infections.

Symptoms
- Sharp pain during and after bowel movements (can last for hours)
- Bright red blood on the toilet paper or surface of the stool
- Fear of passing stools due to pain
- Visible tear or a small lump/skin tag near the site of the injury

Unlike hemorrhoids, fissures cause sharp tearing pain with defecation. Delayed diagnosis can turn an acute fissure into a chronic fissure, which is harder to treat.

Fissures usually occur when the tissue is stretched beyond its capacity. Common triggers include:
